PLEASE NOTE: If you wear contacts and/or glasses, please let us know in the comments. We schedule extra time for contact renewals and schedule an additional appointment with an optician to look at glasses afterward. We want to make sure we can service all your eyecare needs in one visit. Simply add your message in the "Additional Notes" and we'll take care of the rest. We look forward to seeing you!
For existing patients, the online Patient Health Record (PHR) is accessible through our cloud-based Revolution system. If you are new to the PHR, please call our office to get login information. Once you login, you will be prompted to create your own username and password.
